Dexcom CGM: Transforming Diabetes Management Through Data, Quality, and Population Health
Join us for an engaging discussion on how Dexcom CGM is helping healthcare systems improve diabetes outcomes through actionable glucose insights, quality-focused initiatives, and connected technology solutions. This session will review the latest clinical and real-world evidence, including growing evidence supporting earlier CGM use in type 2 diabetes, as well as implementation experiences across integrated healthcare systems, including the VA. Attendees will explore the impact of CGM on glycemic outcomes, patient engagement, and both short- and long-term diabetes management.
Participants will also examine the evolving quality landscape, including the new 2027 HEDIS CGM Utilization Measure and the expanding role of CGM-derived metrics in value-based care and population health strategies. The session will highlight advances in CGM technology, including Dexcom Clarity® population health insights and Dexcom Smart Basal innovation, and discuss how these tools may help identify care opportunities, support medication optimization, streamline workflows, and enhance more proactive, personalized diabetes management across diverse patient populations.
